Is it conceivable that someone could receive a heter today to marry a second wife while actively married to a first?

Well a heter meah rabbanim works that way, but absent that, no. Assuming the person is not from those limited communities that did not accept the prohibition of marrying multiple wives in the first place, no, there is no heter.

Which communities didn't accept the prohibition?

Sefardim. In their Kesuva, however, they have an extra clause where the Choson agrees not to marry additional wives.
