L'Chvod Rabbi Shapiro, Shlita:
If a person is Davening either for someone's Rifuah Shilaymah Mihayrah, or for another type of request
(e.g. Shidduch, Zera Shel Kayama, Parnossa),
whether in Shmoneh Esray or, for example, in Tehillim, Shira HaShirim, or Perek Shira:
1) Should the request be mentioned only once, in one prayer?
Or should the request be mentioned again, every time the person prays,
until the request is fulfilled, IY"H, or until the person doesn't need it anymore (e.g. C"V a Choleh is Niftar) ?
2) Can a request for ongoing good health, long life, and Parnossa be made, every time a person prays?

1) Keep mentioning it as long as its needed.
2) Prayers for general needs should not be added to our Tefilos. That's what your generic Shemonah Esrei is about. We don' tneed to add anything to what the Anshei Kneses HaGedolah saw fit for our general needs. We add a prayer when there is a specific, exceptional need. Absent that, we pray what people bigger and better than us prepared for our needs. (See Aruch haShulchan.)
